Preoperative Diagnosis: Vaughan-Jackson Syndrome. Unable to extend the ring and small fingers. Painful prono-supination.

Operative Procedure: Reconstruction of the Extensor Mechanism. Tendon grafts to the EDC and EDQ. Aptis total distal radial ulnar joint (DRUJ) replacement.

Findings: Subluxation of the DRUJ. No cartilage on the ulnar head. Attritional rupture of the EDC and EDQ tendons.
